Medical records from the prior practice location may be requested through a third-party medical records company called HealthMark Group.

Ready to request your records:

• Click Link: https://requestmanager.healthmark-group.com/register
• Enter your email address where prompted. You will be emailed a secure link.
• After receiving your secure link, submit your record request promptly, as the secure link will expire within 15 minutes.
• Select Medical Records
• Complete all fields under Medical Records
• For Facility that currently has your records, enter:
  • Rheumatology Associates, phone 214-540-0700; 8144 Walnut Hill Lane Suite 800, Dallas, TX 75231
• Who would you like your Records disclosed to?
  • Select Self
• What type of Records would you like?
  • Select Range: Request records for the past one year. If your last appointment date was more than 1 year ago, request records from the date of that visit until the present date.
  • Select Medical [all]
• Complete the authorization page and submit your request.
• HealthMark will send you an email when your records are now available for download. You will be able to download the records electronically.
• Once your medical records are received, you may send them securely to North Texas Center of Rheumatology via email at info@ntcrcare.com or fax at 469-565-9885.
