To ensure a smooth check-in experience, please review and complete the following steps before your visit.
Pre-Appointment Checklist
1. Complete New Patient Forms
- Fill out your new patient forms online at least 24 hours before your visit using the link provided via email or text.
- If you cannot complete the forms online, arrive 30 minutes before your appointment to complete them in person.
2. Bring Necessary Documents
Please bring the following to every visit:
- Photo ID
- Insurance Card(s)
- Medical records (if available)
- Referral for all HMO plans (if required by your insurance)
If your insurance plan requires a referral, contact your primary care provider to ensure a valid referral is sent to our office at least 24 hours before your appointment.
We may not be able to see you if a referral is not on file.
Payment Options
We accept the following payment methods for any balance due at the time of service:
- Cash
- MasterCard, Visa, American Express, and Discover Card
Cancellation & No-Show Policy
Notification Requirements
- If you need to cancel or reschedule, please notify our office at least 24 hours in advance.
- For Monday appointments, cancellations must be made by 12:00 PM the previous Friday.
No-Show Policy
- If you fail to cancel your appointment twice within a 12-month period without proper notice, you may be discharged from our practice.
- New Patients: If you miss your appointment without providing proper notice, we may not reschedule your visit.
